Information concerning the processing of personal data

Dear Sir or Madam,
According to art. 13 of the General Data Protection Regulation of 27 April 2016 (hereinafter referred to as “GDPR” in Polish “RODO”), we inform that:
1. The administrator of your personal data is the Andrzej Frycz Modrzewski Krakow University with its headquarters at Gustawa Herlinga-Grudzińskiego 1, 30-705 Kraków.
2. The University has appointed the Personal Data Protection Inspector, who you can contact in case of any questions or comments regarding the processing of your personal data, at Gustawa Herlinga-Grudzińskiego 1, Kraków or via the e-mail address: iodo@afm.edu.pl.
3. Your personal data will be processed in order to fulfill the legal obligation resulting from:
1) the provisions of the Law on Higher Education (in the case of candidates for studies, students and post-graduate students) and
3) the labor code (in the case of employees),
4) from concluded civil law agreements, in order to fulfill legal obligations arising therefrom, in the case of entities cooperating with the University on the basis of civil law agreements.
4. The University will process your personal data:
1) in the case of candidates for studies based on the consent for the processing of personal data, pursuant to art. 6 par. 1 lit. a, RODO;
2) in the case of students and students for the purpose of providing educational services based on the provisions of the Act of 27 July 2005 – Law on Higher Education (Journal of Laws of 2018, item 730) and expressed by your consent , pursuant to art. 6 par. 1 lit. a, lit. b, lit. c RODO;
3) in the case of employees for employment based on art. 221 of the Act of 26 June 1974 – Labor Code (Journal of Laws of 2018 item 917), pursuant to art. 6 par. 1 lit. b, RODO;
4) in the case of entities cooperating with the University on the basis of civil law agreements based on the consent in the agreement for the processing of personal data, pursuant to art. 6 par. 1 lit. a, lit. b RODO;
5) in the form of the image monitoring image for the purposes of ensuring the safety of persons and property, pursuant to art. 6 par. 1 lit. c and lit. f RODO.
5. Your personal data may only be disclosed to:
1) the employees and associates of the University authorized to process personal data in connection with the performance of official duties,
2) suppliers of technical and organizational services for the University to the extent necessary for their implementation, but only as part of authorizations or assignments (in particular suppliers and entities specialized in providing technical support for ICT systems, monitoring system);
3) entities entitled under the law.
6. The University does not transfer your personal data to recipients located outside the European Economic Area (European Union countries and Iceland, Norway and Liechtenstein), with the exception of participation in the Erasmus + programme and other employee and student exchange programmes.
7. The University will process your personal data in the time necessary to complete the ranges of the offer selected by you according to the agreement and will store your personal data for the period required by law for archiving purposes and possible settlement of mutual claims.
8. The data processed as part of the monitoring are in such a way that the preview of the recording is current and is saved, and the data storage period is about 2 weeks, after which they are removed, subject to situations requiring conducting explanatory proceedings due to security incidents and property. For such exceptional situations, the processing of monitoring data will take place adequately to the existing circumstances and legal requirements.
9. You have the right to access your personal data and, subject to the law, the right to rectify, delete, limit processing, the right to transfer data, the right to object to the processing.
10. You have the right to withdraw your consent at any time without affecting the lawfulness of the processing, which was made on the basis of consent before its withdrawal.
11. You have the right to lodge a complaint with the supervisory authority.
12. Providing your data is voluntary, but necessary to take advantage of the University’s educational offer.
13. Your data will not be processed in an automated way and will not be profiled.
